That I the undersigned, a private citizen, in and for the consideration of the privilege of performing voluntary animal care for Saving Dogs' Lives (abbreviated as SDL) and recognizing the routine animal care activities involve certain inherent hazards, do hereby agree to assume the risk attendant to such activities, to include (as example but not limited to) being scratched, bitten, or tripped, request to work on:

Once prior approval has been obtained, state: I release SDL, its agents, officials, employees, and other volunteers, in both private and public capacities from any and all liability, claims, demands, or causes of actions which may arise from voluntary animal care on a routine basis for said rescue organization.
